"This was a remarkable case. Due to the more homogeneous pressure distribution and lower predicted risk of conduction delay and paravalvular leak (PVL), we decided to use a 34 mm device. The result was astonishing. The valve was deployed at a target depth of -2 mm and it expanded just as predicted. No PVL, no conduction issues! Initially, I was rather skeptical, especially because of the pronounced LVOT calcification and the associated risk of PVL or annular rupture in the worst case.
